Player Justin Bieber who frequents the Chicago Dustbowl server builds teles in such a manner that when his teammates materialize they get stuck. Not a few times but every day for weeks. He is by far the most successful at getting vote banned however he simply returns in 30 minutes to repeat the activity. Accordingly, you may want to consider a permanent ban. To facilitate this I looked for his steam id and was not successful. The log neither shows him joining nor leaving the server. The only evidence he was there is one line of text when he used the chat. I've attached the log for your consideration:
